Noscab contains Permethrin which belongs to the group of medicines called Anti-parasitic agents. It is indicated for the management of scabies, pediculosis (lice), and crab louse infections.
Noscab is not recommended for use in patients who are allergic to chrysanthemums (a flower). Noscab should be used with caution in patients having lung diseases and eczema (skin disease).
It is not recommended for use in infants below 2 months of age and should be used with caution in children and adolescents below 18 years of age. Consult your doctor before using this medicine.

Treatment of Pediculosis (head lice infestation) and scabies (skin infection)
Noscab Cream inactivates the nerve membrane and paralyzes the insects, eventually killing them. It can also kill the nits and eggs of the insect.
Some individuals with scabies may experience mild skin discomfort, such as burning, stinging, or tingling after application. Other temporary symptoms can include redness, swelling, rash, itching, or eczema.
Noscab is mostly used to treat parasitic infections. It targets eggs, lice, and mites.
Permethrin, a pyrethroid, is the active ingredient in Noscab cream, which works as a topical treatment for scabies caused by Sarcoptes scabiei. It affects the nerve cell membrane of the parasites by disrupting sodium channel currents, leading to delayed repolarization and paralysis of the pests.
Pharmacokinetics:
Permethrin is only approved for topical use, and limited human data is available on its systemic exposure when applied to the skin. A small amount (less than 0.5%) of the applied dose is absorbed within the first 48 hours. Absorbed permethrin is rapidly metabolized, primarily in the liver, and its breakdown products are mostly excreted in the urine.

Administration:
Before applying permethrin cream, ensure the skin is clean, cool, and dry. Avoid taking a hot shower or bath beforehand.
Leave the cream on for at least 8 hours before washing it off. Reapply to any area that may have been washed during this time (e.g., after handwashing). If needed, repeat the treatment after 7 days as advised by the doctor.

Corticosteroids used to treat eczema-like reactions should be avoided before using Noscab, as they could worsen the scabies' infestation by lowering the immune response to the mites.

In the absence of specific studies, permethrin should be used in pregnancy only under medical supervision. It is not known whether permethrin is passed through breast milk, so nursing mothers are advised not to use it.
Scabies are often associated with itching, swelling, and redness, which may temporarily worsen after starting treatment with Noscab. Patients should avoid getting the cream in their eyes, and if it does, rinse immediately with water.
